NEW CLIENT QUESTIONNAIRE
The New Client Questionnaire is a quick and easy way for me to gain initial insight into your health history, daily habits, concerns, and goals. It also includes a disclosure and a waiver of liability. You will receive an email with a link to complete this form, along with the Disclaimer of Liability. The email will be from Practice Better, the online scheduling software that I use.
Please complete the New Client Questionnaire as soon as you comfortably can. At the very latest, please submit it at least 24 hours before your consult.
You may want to set aside at least 20 minutes to fill it out during a time when you will have few distractions.
PHONE VS SKYPE
I work best via phone and thus offer all my consultations via phone (or Skype or Zoom). You can read more about 'why' here.
PHONE: I will call you at the time of your consult. Land lines are always best, but if you only have a cell phone make sure you are somewhere that has good reception so we can easily hear each other.
The client portal software I use, called Practice Netter, includes a private and secure video chat feature which is very intuivite to use. I recommend using it.
SKYPE or ZOOM: If you wish to do Skype, please send me a Contact Request via Skype. My Skype ID is tauna.houghton
If you are new to Skype, please do a test run with a friend before our consult to make sure that your video and audio are working well.
As Skype video can slow down the consult with technical difficulties and distractions (some people feel awkward on video), I often suggest that we do the first 5 minutes or so on video so we can greet each other, then switch to phone.
DURING YOUR CONSULT
Please set aside the full time to be in a quiet area that is private and free of distractions.
I highly recommend that you take notes while we talk. In addition, I will email you your customized Nourishing Recommendations after your consult, which will include a summary of my suggestions.
If you need to cancel or reschedule your consultation, please do so at least 24 business hours in advance. For example, if your consult is on Monday at 10am, please cancel by Friday at 10am.
If you cancel in less then 24 business hours before your consult, you will be charged a 50% cancellation fee.

HOW TO PAY FOR YOUR CONSULT
Please pay for your consult beforehand. You can easily pay with your credit card when you book your appointment, or by clicking the payment link in your confirmation email.
